Transportation Forms
Bus Route information will be emailed to parents mid-August.
Field Residents MUST complete a transportation request form located below in order to receive busing services:
This form MUST be completed by FOR ALL students, so your child's school office and staff know where your child needs to go at the end of the day. By filling out this form, it does NOT mean you qualify for school bus transportation. Please complete by Monday, August 3rd.
Please submit the "Alternate Stop Request Form" to the transportation office and allow 5 days to process your request. Your child must be eligible for transportation. Please complete by Monday, August 3rd.
Click here for Field resident students (K-8) who are eligible for transportation that attend St. Pat's ONLY. AM-ride a Middle School route & then transfer onto the St. Pat's bus. PM-ride to Field Middle School & transfer to home route. Please complete by Monday, August 3rd.

PLEASE NOTE - LOCATION CHANGE!
The Maplewood bus will pick up and drop off Maplewood students along the football stadium grandstands.
ALL Maplewood students that drive to FIELD to catch the shuttle bus will need to park in the designated area!
The bus will leave promptly at 7:30 AM and drop off at stadium at approximately 2:50 PM.
*PARENTS dropping off & picking up Maplewood students need to park in spots 88 THROUGH 112 ONLY!
*STUDENTS who park @ Field that ride the shuttle need to park in spots 123 THROUGH 146 ONLY! These spots are located along the grandstands.
The bus will enter from Saxe Rd. and pull directly into this row.
AGAIN: Pick up and drop off location will be along the stadium grandstands.
Students WILL NOT be assigned to the Maplewood bus if parents/guardian choose "no" for transportation or do not fill out this form. The Transportation Department will assume the student's parent/guardian will be responsible for finding alternate transportation if the bus is not needed.
DEADLINE FOR THIS SURVEY IS SUNDAY, JULY 26th.
Times shown are APPROXIMATE.
These times may vary depending on:
1. Weather conditions
2. Start of Kindergarten
3. Students not waiting at their stop 5-10 minutes before bus is scheduled to arrive
4. Changes in routing due to students who move into or move within the district
5. Mechanical failure
Students are NOT permitted to ride any other bus for “social purposes”
Your student must be picked up at the SAME STOP Monday-Friday
Your student must be dropped off at the SAME STOP Monday-Friday
Change of Address
If you have moved within the district, the following must be provided to your student(s)' building secretary before changes can be processed:
Change of address with your child’s attending school main office.
Change of address in Final Forms.
Requests take 24-48 hours to complete. No same day requests.

We are looking for great people to help transport Field Students. Must have a clean background check. We will help train you through all the State requirements.
