Transportation Forms
Bus Route information will be emailed to parents mid-August.
Field Residents MUST complete a transportation request form located below in order to receive busing services:
Please complete the Decline Transportation Form if you would like your child to be a car rider during the school year, or if you have found alternate transportation to and from school.
Please submit the "Alternate Stop Request Form" to the transportation office and allow 5 days to process your request. Your child must be eligible for transportation.
Click here for Maplewood form. The Maplewood shuttle bus leaves the Field High School parking lot at 7:30 AM. In the afternoon, the shuttle will bring students back to Field High School around 2:50 PM. (Starting August 28th and continues all days when Maplewood is in session.)
Times shown are APPROXIMATE.
These times may vary depending on:
1. Weather conditions
2. Start of Kindergarten
3. Students not waiting at their stop 5-10 minutes before bus is scheduled to arrive
4. Changes in routing due to students who move into or move within the district
5. Mechanical failure
Students are NOT permitted to ride any other bus for “social purposes”
Your student must be picked up at the SAME STOP Monday-Friday
Your student must be dropped off at the SAME STOP Monday-Friday
Change of Address
If you have moved within the district, the following must be provided to your student(s)' building secretary before changes can be processed:
Change of address with your child’s attending school main office.
Change of address in Final Forms.
Requests take 24-48 hours to complete. No same day requests.

We are looking for great people to help transport Field Students. Must have a clean background check. We will help train you through all the State requirements.
