Final Forms and Schedule Pick Up

Attention Middle School Parents

This year we will continue to use FinalForms as our online form and data management service. Families must review all
9 of the following documents in FinalForms and re-sign prior to picking up schedules: Contact Information, Health
History & Medical Profile, Medications, Injuries & Hospitalizations, Past & Ongoing Health Conditions, Emergency Medical Authorization, Student Handbook, Student Media Release, and Electronic Device Agreement. Make any and all necessary changes.

If an address change has occurred, please contact the office as well. Please note that schedules cannot be issued until
these steps have been taken.

All incoming 7th graders are required to submit proof of the meningococcal vaccine at schedule pick-up. We also
recommend that you pay school fees during schedule pick-up times or pay online at Pay Schools Central. During
schedule pick up times, students may also find classrooms and practice locker combinations.

8th Grade Washington D.C. Trip (Oct 2019)

All 8th graders will have the opportunity to take a 2 night/3 day trip to tour our nation’s capital October 23-25, 2019. The total cost of the trip is $330, which includes lodging, all meals, transportation, and many extras. Field Middle School staff serves as chaperones throughout the trip. Participation in the trip is voluntary and available space will be filled on a first-come, first-served basis. A payment plan with 2 specific due dates is available. Students who wish to participate in the trip must turn in the parent permission slip and a check for $165 on May 16, 2019.  Please note that early payments cannot be accepted.  If you are considering the trip for your student next fall, click on this link for the informational flyer and permission slip.

Apples for Students at Giant Eagle

Shop. Scan. Support Our School.  Apples for the Students started September 2nd and runs through March 16th.  This program is a great way for our school to earn funds for our schools overall educational needs.  Is your Giant Eagle Advantage Card® registered?  Now’s the time to check.  All you need is the Middle School code - 5608, to verify a card or register a new card.

THREE EASY STEPS:
1. Register your Giant Eagle Advantage Card online
2. Enter your Giant Eagle Advantage Card® number (which appears under the bar code).
3. Enter the school identification number - 5608.

Girl Talk GLITTER

Girl Talk (www.mygirltalk.org) is an international non-profit peer-to-peer mentoring program with a very simple premise: high school girls mentor middle school girls to help them deal with the issues they face during their formative early teenage years. Our mission is  to help young teen-aged girls build self-esteem, develop leadership skills and recognize the value of community service.

Meetings are every Friday from 3:00-4:00

Work Permit Information

 
Work permit information is available at the High School Guidance Office and the Middle School office Monday-Friday 7am to 2pm (starting August 5th.)
